Comprehensive Kidney Care Practitioners In Southern California

The team at Southland Renal Medical Group looks forward to meeting you and serving all of your regular and acute kidney care needs. Whether you are coming to us for a consultation or some type of treatment, we’ll need some information from you in order to get you in our system. This will help us understand your current medical needs and any previous history you have, and help us set up any aftercare you may need, as well.

What you’ll find below is a list of the things we’ll ask you to bring to your first visit to one of our offices. If you’d like a list of our locations, please see the Contact page on this website. Each facility requires the exact same documentation in order to get things set up for you.

Your First Visit

When you come in for your appointment, we’ll ask you for a few specific things that will help us provide you with the best of care. Here is a simple list of what to bring on your first visit to our facility:

  • Picture ID
  • Insurance card(s)
  • Laboratory results, as well as any radiology reports
  • Medications – please bring a comprehensive list of all your medications, including the name, dose, and frequency (or please bring all your medications with you in the original container)

Medical History

Please be prepared to answer questions regarding your past and current medical history, including dates of hospitalization and any outpatient procedures you may have had. If you prefer, you can write your history down ahead of time and bring it with you. We’ll also ask about your family medical history starting with your grandparents, and any history of illness in your family.

All of this information will help us understand the “big picture” in terms of your kidney health. It will also help us to ask the right questions when we’re addressing any of your kidney-related issues. Since family history can play a part in various kidney diseases, we’ll want to gather all the information we can about your background before coming up with a treatment plan.

Aftercare And Ongoing Support

After you are seen by one of our doctors, you’ll be provided with a summary of your visit as well as instructions regarding aftercare and your next visit. We want you to have all the details so you can understand your kidney health and any plans we have for testing or treatment. When you get these documents, feel free to talk with us about anything that is unclear. We’re here to help!
